RSD is a clinically-coined term for sudden, intense emotional pain triggered by real or perceived rejection — and the evidence base is thinner than its popularity suggests.
Not a Formal Diagnosis
RSD isn't in the DSM-5-TR. Psychiatrist William Dodson introduced it clinically in the 1990s based on his own caseload, not a controlled study. His estimates (most ADHD adults experience it; a third call it the most impairing part of ADHD) are clinical impressions, not peer-reviewed prevalence data.
What Makes It Clinically Significant
The speed. RSD can move from trigger to full crisis in minutes. Combined with ADHD's impulsivity, that leaves almost no gap between a thought and an action — the mechanism behind ADHD's suicide risk pathway.
Not Exclusive to ADHD
RSD-like emotional dysregulation also shows up in autism, BPD, and anxiety disorders. Roughly a third of adults with ADHD describe it as their most impairing symptom, even though it isn't a core diagnostic criterion.
The Clinical Ask
Has real or perceived rejection ever triggered a rapid, intense mood shift — closer to physical pain than sadness? Rule out a primary mood disorder before treating this as one.
